Dr, this has been such a fun thread. It is one that comes up every few years, and it is nice to read the stories of some of the newer members.
I've answered this, here, a few times, but just for you, I'll repeat some of my story.
I was just 14 in 1960, and my father, who was a life-long, heavy, cigarette smoker, and occasion pipe smoker, wanted to discourage me from cigarettes. He gave me one of his pipes and a pouch of Cookie Jar. (I didn't want to tell him that I'd been stealing some of his Parliaments and my mothers Kents, for two years.) I took the pipe and tobacco and got hooked. Although the tobacco scorched my tongue, I kept with it, because I loved the smell. One couldn't smoke a pipe around one's peers at 14, without being a target for ridicule in those days, so I only indulged when I was alone.
In my late teens, my father's friend, a committed pipe smoker and collector, became my "guru." He taught me about good tobaccos, how to care for my pipes, and how to pick good, affordable pipes.
Now, 60 years later, I try to be that same kind of "guru" to the newer smokers in our Chicago Pipe Club.

Dino, thanks to you for taking your time to share your account with us yet again! Glad you are teaching your 60+ years old legacy to new members in your club and PM alike and I hope it lasts eternal!..
There are my close friends, my seniors, resident docs in hospital and many others who discouraged me never to hold Cig if you want to live peacefully.. then they shared their stories which in majority holds relations with overcoming stress by risking their health..

"Man, it's better you never start.. I myself wants to give it up.. But just can't.. things don't work easy without this.." Such had been the comments I often hear from them all!

I voluntarily opted to go with Pipe Smoking instead. Reason solely being I love indulging myself into complicated things.. which looks like a puzzle in first glance but as soon as you step forward.. the beauty of the task enhances..
Pipe is a ritualistic activity.. and hope will turn in my favor..

Chopper, finally you did it! Glad you completely gave up Ciggs for Pipe and I'm happy that your pipes and blends are serving your taste buds and nicotine requirements well and in moderation! Ciggs are difficult to give up.. and Nicotine Replacement Therapies can cause huge havoc by giving excruciating withdrawal symptoms to a conditioned smoker.
And I too had fingers crossed for Cherry Blends.. I am desperately waiting for my Missouri Prides which are in transit and may show up by next week..
Health authorities are well known to place any type of tobacco top in blacklist of Oral Cancer Causing Agents as no form of tobacco be it smoking or smokeless can be labelled 'safe' for consumption.. but we all in community knows the real side of Pipe Tobacco.. additive free.. strength ranging from mild to strong in taste and Nic content and plethora..

It's very well known how nicotine can calm and focus the mind. I learnt to treat stress and depression with nicotine from a rather young age. [Smoked regularly from 12yo, daily from around 14yo]
Over 20 years ago I was diagnosed with Buergers disease. Nicotine is said to be the cause.
Before I switched to a pipe I walked a very fine line where if I smoked stronger tobacco my symptoms would flare [Interesting what you said about Bidi's - some here in Oz think it's a herbal smoke but I learnt the hard way that they ARE high in nicotine]

Reading many reviews and comments here about stronger blends, I at first was a little disappointed that they're too high in Vit N for me to try but I soon realised that few who smoke these blends actually inhale which is a reflex for me.
I'm content that there's a vast range for my needs from which to choose.

I've surprised myself that I've been able to maintain the pipe without cravings. Some of my blends have hardly any nicotine at all. Maybe smoking a pipe gives me a placebo effect? Idk but I'm delighted to finally be rid of the cigs once and for all.
At first smoking a pipe was a compromise where I told myself 'At least you get to have a smoke'. I can't remember it being that hard at the time, probably because I was determined. Now I wouldn't smoke anything but my pipe [I even gave up cigars which I really enjoyed on occasion]

Oral cancers from pipes were much more common back when people smoked clay pipes [and when oral hygiene wasn't what it is like today]. Same when smoking a RYO without a filter; if ones lips are dry a clay pipe or RYO paper can stick to ones lip. Pulling either from a lip when stuck can cause a small wound. Tar from tobacco can aggravate the wound leading to a cancer. Same with mouth sores and ulcers.
When I had dental surgery I was careful to avoid smoking or protecting any wound until healed.
As long as one maintains good oral health, the risks of developing oral cancer is very low.

You are going to love your MM cobs [there's some good threads here on breaking in a cob pipe] I've 3 MM cobs that I smoke at least once daily and intend to buy a few more.

Wow, chopper, that's a big accomplishment.

Agree it's almost absurd pipe tobacco is piled in with cigarettes, and sometimes vaping.

For me it was a huge accomplishment because I used cigs as a way to cope with stress and depression for most of my life.
None-the-less, it still surprises me to this day just how easy it was to ditch the cigs for a pipe.
Quit attempts were torture. The longest I went without was 6 months but stress or depression would have me reaching for a cigarette. All it took was one to fall off the wagon.

My success came down to three factors; I was so sick and tired of being a slave to cigarettes. The two blends that I smoked when I switched had a reasonable nic-hit "just past the centre of mild to medium" - JimInks [ higher than the blends that I now smoke] and sheer obstinate determination. A bowl of pipe tobacco was better than going without a smoke [Placebo effect?]
Although I had no mentor and fumbled my way through, it didn't take long before I started to enjoy the pipe way more than ciggies.

If our health authorities actually cared about smokers health, they would be encouraging cigarette smokers to switch to a pipe which is way less harmful.
Most smokers who switch from cigs to a pipe find that over time, they end up smoking far less frequently than when they smoked cigarettes.

Chopper, how are you now? You mentioned you diagnosed with Buergers 20+ years ago.. It has its highs and lows man, as you are fully aware by now!.. generally in my Medicine lectures, I was taught that the only step towards curing the disease is to completely abstain tobacco product consumption from daily routine.
The more sustained amount of stenosis happens to arteries and veins, more chances can have of gangrene to the body extremities. I myself saw patients at All India Institute of Medical Sciences, Rishikesh during my Medicine posting with smoking history of 30+, 40+ years (Bidis mostly!)

Chopper, I'll not recommend you to completely stop your pipe ventures. But yes, a gentle remainder that please to keep things in moderation. I am assured you are practicing that pretty well by giving up ciggs and switching to mild alternatives. It will surely help you to a great extent!

I started in 08 at the age of 40 to help w/ quitting cigs. Between the pipe, snus and snuff, I went from smoking a pack and a half a day to a pack a week in no time. Now, I don't smoke cigs at all and just took up the pipe again after a long layoff. Got sucked into cigars for a handful of years there.

I don't know what it is, but I love the smell of tobacco in the bag, in the tin or in the humidor. Smoking the stuff is only half the pleasure.

Moonbog, here in my place we have a dedicated vintage shops that sell loose tobacco for Hookah, RYO and Pipes. Hookah tobacco of all have a distinct aroma of molasses and is also very gooey in consistency that's been sold there..
Reason why I don't purchase pipe tobacco from there is its flat bland, very dry and holds similar aroma of Cigarettes tobacco.. though being additives free.
I hope real pipe tobaccos are far better then this to turn out in my favor..

I don’t have a great story, no one in my family, I mean no one smoked a pipe but growing up I always was attracted to the aroma of a burning pipe. My brother and I would always go in the Tinderbox in the mall to enjoy all the smells and I remeber thinking to myself, “someday I’m going to smoke a pipe!” It was just a fact, I knew it would happen, the question was, when would I be ready for the pipe?

I tried it on and off throughout the years but I always went back to dipping and/or smoking cigarettes. Finally, the stars aligned, my father in law was an ex-pipe smoker and he gifted me about 5 beat up basket pipes and a pack of Captain Black and I was set. It almost didn’t stick though, while I do absolutely love the alluring aroma of Captain Black it was just too light for me, so as with anything in life, if you want to find the good stuff you gotta search for it... Once I discovered English blends, it was over and I was bit by the pipe smoking bug ever since and the rest is history.

I started in September 2009 but I really went down the rabbit hole when I bought my first brand new pipe in Feb of 2010 so it took me about 5 months to be totally smitten by it. Pipe smoking is one of my greatest pleasures and I couldn’t imagine being without it.

Well okay, since you're a medical doctor you might well find this interesting. Long story.

Over 20 years ago I went to my GP with a sore foot. He sent me straight to a vascular surgeon who sent me straight to a hospital bed & a morphine drip. I quickly deteriorated and the pain was insane. My foot could not even tolerate the weight of a bed sheet.

An angiogram showed that I had zero blood flow to my left foot and 30% to the right.
The surgeon said 'Your hands are also compromised but we didn't bother to check because it's an unpleasant procedure and since you'll be dead soon we didn't want to distress you further.' How's that for a bedside manner?
They said "100% of Buergers patients will suffer multiple amputations and death within 2-3 years after diagnosis."

I'm not frightened of death but my boy was quite young so I did a lot of research regarding vascular health.
You might be surprised at what I found. The main cause of the modern rise in arteriosclerosis is margarine and hydrogenated vegetable oils. They're also a big contributor to the rise in diabetes.
I'll only eat butter, ghee or cold pressed veg oils [I'm a vegetarian]

At the time I was diagnosed I was going through family court with a an ex-wife who took great delight in making my life as miserable as possible. I've never been so stressed in all my life.
The more stressed I was the more I smoked. It was a combination of stress and nicotine that caused my illness.

For ten years after my diagnosis I walked a fine line with nicotine. Fortunately we have mild Winters in Oz but it's cold enough to mess with me. Every Winter I need to remind myself that it's imperative that I must do certain things. Like washing my hands in warm water. Be inside a warm house when the temp hits 10*C no matter how well I'm rugged up.
If I'm not careful, my foot and hands get so cold that they remain that way until Spring.

Over that ten years I quit cigarettes so many times. Stress or depression would knock me from the wagon. Once I learned that pipe tobacco had no added chemicals, unlike cigarettes, I was determined to switch to a pipe.
Now I only smoke blends with mild to little nicotine.

I've seen over 15 of my past lives and have helped others to witness hundreds of theirs. There's no doubt in my mind that we live life after life so my attitude is that smoking a pipe is one of my last remaining pleasures in life so buggered if I'll deprive myself.

Buergers is a cruel and painful disease. Because I refused an amputation for months after a by-pass failed, the surgeon said if we waited any longer he would be forced to take it off above the knee so I agreed to a below knee amputation. [Although I'd had zero blood flow to the foot for 5 months, it had not turned gangrenous or black, just a deathly white]
The pain specialists prognosis was that because I had endured a high level of pain for so long, the nerves would 'hold the memory'. They were right.

After the amputation a nerve grew back into a ganglion of nerve endings [neuroma] that is the cause of excruciating nerve pain. Fortunately the pain was not constant 24/7 but when it hit, the pain would literally lift me out of my chair. Like being repeatedly stabbed with an electrified needle.

The pain specialists tried to force me to take an anti-epileptic [Epillim] for the nerve pain because 'It might help'. Bullshit it does.
The side-effects make one feel like crap, it slowly destroys the liver and sudden cessation can kill. It's a very dangerous drug. These day they prescribe Lyrica which is just as bad.
Just 2-3 pipes of cannabis a day eliminates at least 95% of the nerve pain. As a happy consequence, I learnt that cannabis has an anti-inflammatory effect that has slowed or stopped the progression of the disease.

I've known 12 other Buergers patients over the last 20 years. They all suffered multiple amputations and death within 3 years after diagnosis.
They all put their faith in the specialists and took all the prescribed medications. Epillim or Lyrica, Prednisone and a synthetic opiate such as oxycodone or methadone. All of which I refused. My belief is that these medications hastened their deaths.

What's really disappointing is that were I in my doctors shoes, I'd want to know what my one surviving patient was doing differently while all my other patients were dropping around me like flies. Apart from my GPs none could care any less.

Cannabis has not only spared me from excruciating nerve pain, it's literally saved me from a rapid decline. It's an outrage that so many people are suffering unnecessarily due to a corrupt system that keeps an extremely safe and effective medication from patients.

I'm under no illusions. Buergers will shorten my life. It would not surprise me if I undergo another amputation before I die. More than likely above the knee of my amputated leg as it causes me quite a bit of pain.
Learning several forms of meditation has helped me to distance myself from pain to some degree. The open-eyed meditation technique as taught by the Brahman Kumaris in particular.

So until then I intend to enjoy what time I have remaining. I'll puff my pipe without any fear and with no regrets. :col:

Problem is whatever medicine you try to inject into your system it does alleviate your pain for a while but will surely generate another unwanted symptoms along with. General Physicians are like close members whom you can trust and share your dealings. They hear you properly, they suggest you properly and they'll never run for your money.
Specialists on the other hand are mainly focused with particular region to deal with.. this generally eliminates their management scope of emotional dealings of a patient who's in excruciating pain.

Cannabis is not truly introduced to modern medicine but if we look for an alternative means.. Cannabis is a wonder drug that hold good response to relieve various medical conditions..

Thanks for your kind words Dr but no need to feel sorry for me. We all have our crosses to bear in life.
We choose our circumstances before we come into this life. Nothing happens without 'permission' at some level and everything happens for a reason.
The physical world is an illusion and this life is just one of many.
I'm clearing karma from past lives so that I may progress to a higher realm in my next.

I'm fortunate to live in a country where I'm looked after with a nice home and reasonable disability pension.
I've recently fostered a 2 1/2yo German Shepherd with a terrible history of abuse who I'm rehabilitating. He has come such a long way in a short time.
I'm able to help others as a clearer of negative entities [the religious call me Exorcist] and I've helped hundreds to see their past life memories. Both activities bring me much satisfaction. I also teach meditation.

I've learnt so much in this life and have had so many unique experiences that has enabled me to see things from a much higher perspective than most.
Best of all I got to help raise my son to be a fine young man who is now studying business law at Uni.
I have much for which to be grateful.

When one has been through the medical system for as long as I, you get to meet others who have it much tougher. Like the 45yo mother who attends the same prosthetic clinic as I, who had all four limbs amputated after contracting meningococcal at 35yo.

You're spot on regarding the differences between specialists and GPs. Most of my GPs have been very supportive and non-judgemental of my choices.
Yes, pain medication is a tough one to navigate with long term pain. At first I didn't cope so well with my diagnosis and was taking over 500mg of morphine daily.
Long story short I woke up to myself and am now on under 150mg daily.

The U.S. opiate crisis has created waves here in Oz so the pain clinic is under pressure to reduce patients or switch them to methadone which I refuse [It's a slight of hand - patients who are prescribed methadone go from the 'Post-op patients who require opiate medication' list to 'Post-op patients who've become dependent on opiate medication'. The former classification makes the Dr's look bad, the latter puts the blame onto the patient. I'm not a junkie so I refuse to be turned into a drugged-out zombie]
Interestingly there's been a change in the pain clinics attitude towards cannabis at my last appointment. I'm hoping to get official authority to use it as part of my treatment plan.

Interacting with the spirit world has brought me many bonuses in life. For example; before the laws changed here regarding the importation of tobacco, there were rumours that it would only affect 'commercial quantity'.
But I had a really bad feeling, so I ordered as much tobacco as I could afford. Other Aussies on social media bagged me out for being 'paranoid' when I advised to stock up.
As it turns out my course of action was correct. I've got at least 35lb of tobacco 'cellared'.
Most of my blends have mild nicotine levels so I can smoke my pipe without fear of exacerbating my condition.

Thanks for your kind words Dr but no need to feel sorry for me. We all have our crosses to bear in life.
We choose our circumstances before we come into this life. Nothing happens without 'permission' at some level and everything happens for a reason.
The physical world is an illusion and this life is just one of many.
I'm clearing karma from past lives so that I may progress to a higher realm in my next.

Chopper..I am happy you are rehabilitating your dog. This will ultimately help both of you to overcome your shares of dark sides of this very life. Pets are amazing you see.. they hold no expectations (cats aside ?) and only gives you love in return.
By reading your reply I can easily sense your are overcoming your illness pretty well. Keep up the great work like this..
And here in India We all are celebrating Holi Festival today.. it's a festival of Colors. And owes a significance to eliminate all the dullness in one's life and to add up colors and happiness onto the former.

I've been drawn to pipes since a very early age.

Why, that is hard to answer.
No pipe smoking father, grandfather or uncle.
I guess there was some kind of atmosphere around the pipe that was appealing to me.
A sense of calm, order, sophistication, independence.
Maybe it also was the environments in which pipes often were seen; offices, desks, book shelves, libraries.
I also thought the pipe was a beautiful and interesting object in itself.
Somehow it caught my attention and still does, decades later.
