Smoked cigarettes for I don't know how long. At first, retrohale didn't work for me. It didn't bother me, but I didn't pick up any notes. After 3 yrs, I'm starting to get some notes. Orientals are nice, so do the dark fruits. Still get most flavors from tongue, but nice to notice my nose working again.

I drift the smoke out of my nose most of the time. Also, I just breathe in and out of my nose while a wisp of smoke comes from my mouth. I will also breathe lightly through the pipe and exhale from my nose. With all of those 'methods' I feel like I'm getting the most the tobacco has to offer, on all fronts.

Nicotine is a vasoconstrictor. I wonder if the capillaries in your sinus tract are constricting and giving you a sinus headache? Probably just temporary until your vessels get acclimated but hard to say.

I retrohale more often than I exhale through my mouth and I've never had problems with headaches.

What kind of tobacco was it? I’ve had irritated sinuses when I try to retrohale a lot of dark fired.

You’re not actively blowing smoke out your nose are you? That will hurt. You just sort of let it drift out the same way it drifts out of your mouth.
I believe that it was a Comoy aromatic.
