Maybe. The Ajustomatic tenon is made so that the part that fits in the stem has an area with a larger diameter than the rest of it--has a hump in the middle. The stem is heated and the tenon pushed into the stem. When it cools that raised area locks the tenon in place and allows the Ajustomatic to work. The replacement stem must be drilled to the smaller diameter on the section inside the stem. You then heat the stem and push it in. The heating and inserting the Ajusto usually causes the stem to swell a little which means it must be sanded down to match the shank diameter. I've done this several times and have even used the Ajustomatic in a couple of the pipes I've made.
