If it's a work call, I don't smoke. Plain and simple.
I can tell if the other person on the line is smoking a cigarette, and I consider it rude when they smoke and talk to me through work, but if it's a friend, I don't mind. I smoke my pipe with friends and family because the know I smoke, so they already accept it...for everyone else, I let the bowl go out.

The only time I've ever answered the phone while smoking was when my mom called a few months back. Fortunately, the "conversation" tends to be a little one-sided, so I was able to hang onto the phone with one hand, and remove the pipe to make the occasional reply with the other. The pipe eventually went out (my mom can talk the legs off of several mules), at which point I set it down regretfully and waited for the end of the call to relight, tamp, etc. - can't do all of that one-handed yet. :D
