Yes, it can be fixed, but I'm not clear enough to relate how.

Hi fellow pipe smokers,
I was wondering if there is any way I can line up my stem (that screws into my bowl Turkish Meerchaum pipe). When I carefully tighten it up, my stem lip is off to the side. Any ideas how it can be adjusted? Please let me know. Thank you.

If it is a screw tenon use a few paper rings or a felt ring
Otherwise paint the tenon with a couple coats of clear nail varnish. Take care to let the varnish dry completely before reassembling the pipe

Does it screw directly into meer or into a nylon insert? I had a similar problem until the thread got gunked up and I accidentally pulled the insert out - blessing in disguise, because now it's like a conventional stem and lining up is no issue.

You removed the shank insert didn't you?

If this is what you have,

twist it back into the pipe and continue turning the stem clockwise to remove the stem without removing the threaded insert.

If the tenon is metal, slowly heat the tip of the tenon with a lighter to soften its adhesive within the stem and carefully twist it to fit.

The threads in the meerschaum will eventually wear down with repeated insert removal.

When I make a Meerschaum pipe I never use threaded inserts even though I can buy them. I use a larger diameter piece of Delron to line the mortise that the tenon will actually go into. That way there is no wear on the raw Meerschaum or worrying about alignment.
Just my two cents...

Just my two cents...
After having to pay to have a permanent fitting repaired, I'll never buy another that doesn't have the screw in inserts. No lead time, can be done at home, and costs considerably less to repair.
