If the pipe gurgles, I fix it (or try to). Usually it's just a matter of opening up the airflow in the stem, especially at the bend (if there is one), and funneling out the last half inch leading up to the button. Pace Rick Newcombe, I almost never fool with the smoke hole through the shank -- I think I only have one or two pipes where this was necessary.
But in the moment, if there is a gurgle, which happens in cool, damp weather or if I pack too tightly, I just take out the stem and blow the moisture out. Harris is right, though, you should never force it out (or especially back in!), but I often sand my tenons down just a hair so that they can be removed and reinserted without much force. Overly tight stems worry me in general, and it's really not necessary for them to fit overly tight to have good airflow.
