I smoked inside for years. But the last few years I decided to smoke outside. This was after a remodel and new furniture. My wife is ok with me smoking inside, but it seems mean because she's a non smoker.
I actually prefer smoking outside.
When it gets too cold, I put the pipes away for the winter. This usually happens the end of October, or early November up here. I can handle 40 degrees, but once it hits 30s, im done until Spring.
Its almost time to put the pipes away for the season. Im trying to get through a few open tins first.

I smoke on the patio or the front porch pretty much exclusively. When I do smoke inside, it’s at my friend’s house while watching college football or a film, but because it’s not my own place, it’s still a bit weird.

I have to do a bit of work this weekend to get our garden shed set up as a smoking lounge that I’ll move an easy chair and small room heater to for the late fall/winter smoking season.

It can get pretty cold here in northern Indiana (several days below zero each winter), and those are days when even several layers of coats and hats and heated gloves are not enough, so nasal snuff becomes my preferred nicotine delivery system.

I smoke both inside and outside but the most outside. I am lucky I live in Southern California in an area with mostly no wind and no rain and I love to walk outside. Definitely I do not smoke outside if windy. The trick for me when walking and smoking is just to stop a couple of seconds for puffing sipping slowly which makes me appreciate the taste of the tobacco. It gives the time to look around at the nature and meditate and I do not overheat the bowl. So when smoking the walk is definitely slower but relaxing and meditating. And a plus is that outside it requires less relights probably because just the movement of the air while walking keep the tobacco lit.
If the weather is not good or I am not in the mood to be out I smoke inside.

I like to smoke indoors, and keep the window slightly open, about 20/40 centimeters, but if I smoke outside, the wind speed has to be almost zero, unless I'm on a terrace and a little protected from the wind. In short, I like the concentration of aroma, so I prefer to smoke indoors. Now that I've lost my sense of smell, I prefer to look at the tobacco and suck on a lollipop.🥴😵‍💫
I hope that doctors find some way of helping your situation!

Outside I smoke faster, I pay less attention to the simple enjoyment of the tobacco, unless it's sitting around the camp fire with my buddies. Inside I savor the bowl, enjoy the different levels of the tobacco I'm smoking and it's the only thing I'm doing. Outside I smoke simple burleys, codgers and shag, a straight up "working" tobacco. Inside is where I break out the VaPers and the flakes, the serious tobaccos.

All my smoking has been outdoors except on really cold days when I don’t smoke. Recently I tried on 3 different occasions smoking indoors. Was not as enjoyable or flavorful, so this Bob agrees with @anotherbob and @Day2Day that outdoors tastes better. puffy

I'm in the same camp even a baloney sandwich you walk out on the back porch to eat taste better.
The best baloney sandwich I've eaten was the first one eaten while camping. The best smokes of any kind are around a campfire after a day spent hiking.

It took me too long to realize that if I can't enjoy it don't smoke. Which when it's too cold I can't.

It is not for me either. For one thing all the indoor smells interact with the tobacco and many of the indoor smells aren't the greatest. Dust, chemicals, people, etc... even the good indoor smells compete with the tobacco. Where as many of the outdoor smells go good with tobacco. Passing near blooming jasmine flowers at night compliments pipe smoke more then it should.
Though I have to admit the novelty of smoking indoors tickles me. But that tends to be social situations where I will suddenly care about room note.
