Try removing and replacing the stem by turning it clockwise only. Turning counter clockwise will either cause the stem to unscrew from its insert, or cause the mortise insert to unscrew from the meerschuam.

Thanks! Had no idea I've been removing it wrong this whole time using a meerschaum I thought the stem was just supposed to screw on it

Yep. Clockwise is the only direction you should ever really need to spin the stem. You’ll probably eventually wear out the meerschaum threads in the stummel if you keep doing it the other way.
