For severe, intractable pain, and especially for dying cancer patients it replaces expensive opium poppy based narcotics, since about 1968. It’s dirt cheap, and replaces much more costly substitutes.

You aren’t going to be able to buy it legally without a prescription and you won’t get it prescribed unless you are in a bad, bad way.
The world cannot uninvent fentanyl.
