Tin: goodness...... Not broken down by style of blend? Well, if I had to pick only one: Escudo. But I could name about a dozen runners up :)
Bulk: Stokkebye's Luxury Navy Flake, again, with about a dozen runners up.

Well in the UK you can get G&H and SG in pouches. So I guess this would be an american survey.
Best bulk in my opinion would be El Ricon de la Pipa #1, it's only available in bulk form too.
P.S flakes would be subjective as they actually come in boxes.

Best is too subjective to answer, but my current favorites are:

Pouch - Mac Baren Cube Silver

Tin - Samuel Gawith Balkan Flake

Bulk - Whole Mexican cigar leaf for blending
All three will change as my taste and experience evolve.

I'll go with Condor for the pouch (even though I've never had any I have no reason to doubt it's good).

Similarly Ennerdale is my vote for the tin (this I have had).

Then pick up some Samuel Gawith St. James Flake in a 250g box for the best bulk.
